Enhanced Care Management
Coordinated support that helps eligible participants navigate healthcare, behavioral health, housing, recovery, and community resources.
What is Enhanced Care Management?
Enhanced Care Management (ECM) is a whole-person care management benefit under California's Medi-Cal CalAIM framework. It addresses the clinical and non-clinical needs of high-need Medi-Cal beneficiaries through intensive, face-to-face care coordination.
As a subcontracted community-based ECM provider in Santa Clara County, Mission Possible works closely with lead ECM organizations, managed care health plans, health systems, and county partners to break down barriers to care.
Mission Possible serves as an Enhanced Care Management subcontractor for Santa Clara County Health Plan.
We help eligible participants navigate healthcare and community systems while addressing needs that can affect health and stability — including care coordination, healthcare navigation, behavioral health connections, housing-resource navigation, transportation, benefits, and social-needs support.
